Did you have any luck with this?
I have a Model B+ and this has started happening recently to me as well, it seems to happen any time the Pi is processing and it stays on all the way through a video. Using XBMC, it appears to overlay the GUI

The package called 'raspberrypi-firmware-bootloader' causes it. I tried to use the bootloader files from here, but the bug appeared again. I don't know which updated file makes it, so I'll look for it.

@pagnol: Try to downgrade the bootloader or overwrite the current bootloader files with the original 'not upgraded' ones. I have some business in the next days, so it'd be cool if we could help to eachother. ;)

The files in that package come direct from here https://github.com/raspberrypi/firmware ... aster/boot
We don't compile, they are binary blobs and you should report to issues on that've site

(Passing this along because I saw pagnol's question on RPi firmware. Don't want to steal his thunder here ;) but thought the info would help)

It's an undervoltage check that was built into the B+: http://www.raspberrypi.org/forums/viewtopic.php?f=29&t=82373:

You can disable it:

$this->bbcode_second_pass_quote('', 'A')s usual you can override the behaviour in config.txt if you understand the risks:
avoid_warnings=1 removes the warning overlay.
avoid_warnings=2 additionally allows turbo when low-voltage is present.
