Thanks for your help Orbi, figured out my stupidity.

For anybody else having a similar problem that...
$this->bbcode_second_pass_code('', 'ir-keytable -t')
Is not generating any events at all. Make sure Kodi or xbmc service is STOPPED.

You need to stop the Kodi service for "ir-keytable -t" to generate events.
$this->bbcode_second_pass_code('', 'sudo systemctl stop kodi')

Ah yes, indeed. I didn't mention this because I kind of assumed you knew this from the start.

You can check what process is keeping the remote busy:
$this->bbcode_second_pass_code('', 'fuser -v /dev/input/event0')
