$this->bbcode_second_pass_quote('WarheadsSE', 'I') have hit this once or twice, but only when copying hundreds of files.

This issue happened to me on both ntfs and ext3 drives, and I found the fix. The problem is with smb.conf in the global section, and something in the following settings fixes it perfectly.

I don't know which lines specifically fixed it, I just changed to the above example I found elsewhere and bingo, no more problems.

Sweet! Good job. My guess is that oplocks = No made the difference. Check your samba status. Seems like some variables are wrong.

I've used munin before but that things is a resource hog. I wouldn't recommend any to be honest not to waste resources that can be used for other things.
